Annual Reporting Requirements for ERISA
The Form 5500 series is required to satisfy the annual reporting requirements under ERISA (the Employee Retirement Income Security Act of 1974) and the Internal Revenue Code. The Form 5500 series includes the Form 5500 as well as additional schedules, such as Schedule A. Schedule A applies to insured plans and includes details regarding premiums and commissions paid during the benefit plan year.
Plans with more than 100 participants are generally required to file the Form 5500 series on the first day of the plan year. Unity Health Insurance automatically generates the Schedule A 5500 information and mails it to the employer group for those groups with memberships of 75 or higher.
