Unity Health Insurance (Unity) has been proud to give you access to high quality health care and dedicated customer service. In order to serve you better now and in the future, we have joined forces with Gundersen Health Plan, and on October 1, 2017, we are changing our brand names to Quartz.

Quartz Postcard

During the next year, when you renew your plan or request replacement ID cards, you’ll receive new ID cards with the Quartz logo. Until that time, continue to use your ID card with the Unity logo.
