If you’re new to Unity, your first step is to choose your doctor, known as your Primary Care Physician (PCP). PCPs manage your care and lead your health care team to help make sure you’re getting the right care at the right time.

Visit Find a Doctor to choose your PCP. Be sure to have your Unity ID card with you so can find the right network. Once you select your network, you will be able to select your doctor based on name, location or specialty. If you do not select a PCP, Unity will assign one to you.​

Access to UW Health

If you’d prefer a doctor (PCP) from UW Health, just contact the UW Health Welcome Center. This is a service that helps new Unity members choose a UW Health Clinic in Dane county.

Types of Unity Insurance Plans

No matter what kind of health care you need, it’s important to check what your plan covers. Unity offers a range of plans:


Your benefits differ, depending on the type of plan you have. A referral or prior authorization may be needed before some services will be covered by Unity. Your Unity ID card will state what kind of plan you have. Be sure to check coverage before getting medical care other than from your PCP.


Unity does not pay for every kind of medical care. Some medical care needs prior authorization. This means that you or your doctor must get approval from Unity before the service is provided.

Determine costs you may incur prior to receiving health care services using the Determination of Benefits FormMyChart link. To complete the form, you will have to ask your provider for certain medical coding information including procedure codes, procedure modifiers and unit codes for the services.
