Your Plan Options

Quartz offers four types of plans to choose from. Depending on the type you choose, the plan pays on average, for a certain percentage of your covered medical costs** –

  • Gold: 80%
  • Silver: 70%
  • Bronze: 60%
  • Catastrophic: Variable

All Quartz plans provide a set of preventive care services without a copayment, coinsurance or deductible when provided by in-network providers
