Important Information About the Quartz® ID Card
IMPORTANT! The third panel of the ID card includes information for both participants and providers.
In particular, the Prior Authorization Information is very important. In some cases, the provider, patient or family member must call Customer Service to receive certification of certain cost management services. This call must be made at least three days in advance of services being rendered or within 72 hours after a Medical Emergency. Failure to follow this procedure may reduce reimbursement in the form of a penalty or a denial of services.
