Terry Bolz, President and Chief Executive Officer

Terry Bolz serves as the President and Chief Executive Officer of Gundersen Health Plan, Unity Health Plans Insurance Corporation and SPWI TPA, Inc. dba Quartz®, co-owned by UW Health and Gundersen Health System. Bolz is responsible for overall executive leadership of the three organizations, with a focus on providing access to local health care providers, high quality and cost effective health care and outstanding customer service.

Bolz served as President and CEO of Unity from 2004 to 2016. He also led Unity from 2000 to 2004 as President and CEO (when owned by Cobalt Corporation). Bolz served as Vice President of the North Central Region of Blue Cross & Blue Shield United of Wisconsin in 1999 and early 2000.  Prior to joining Cobalt, Bolz was a Vice President with Wausau Insurance Companies.

Bolz holds a BA in economics from Lawrence University and the Certified Employee Benefit Specialist (CEBS) designation. He also completed the Nationwide Insurance Leadership Development Program.

Bolz has been involved in several industry associations, including serving on the Executive Committee and Board of Directors of the Wisconsin Association of Health Plans (WAHP) and Board of Directors of Wisconsin Health Information Organization (WHIO).  Bolz is also on the Board of Directors of the Boys and Girls Club of Dane County.


Gary Lenth, MDChief Medical Officer (CMO) and Executive Vice President

Dr. Lenth provides leadership to the combined organization of more than 269,000 members as Chief Medical Officer and Executive Vice President of Administration. He is responsible for health plan clinical services programs, including medical, pharmacy, and quality management programs.

Previously, Dr. Lenth served as CEO of Gundersen Lutheran Health Plan since 2012 and has been its Medical Director since 2007. His service included membership on the Gundersen Lutheran Health System Board of Governors and the Board of Trustees and the Health Plan Board of Directors. He served on the Board of Directors for the Health Plan Alliance and for the Wisconsin Association of Health Plans.

Dr. Lenth continues to practice in the department of Ophthalmology at Gundersen Lutheran Health System as he has since 1988.

Dr. Lenth earned a Bachelor of Science from Iowa State University, and his Doctorate in Medicine from the University of Iowa Medical School, in Iowa City, Iowa.  He completed his Residency in Ophthalmology at the University of California, Davis. He is a member of the American Academy of Ophthalmology.

Marybeth Bay, Vice President and Chief Information Officer

Marybeth Bay joined Unity Health Insurance in 2016. Previously, she led information technology and initiatives for several health care organizations.

Bay provides leadership, direction and administration to the combined organization's information technology services. As a member of the Executive Team, Bay is responsible for the development and implementation of the overall information technology services strategy.

Bay earned a Master's degree in Health Administration-Informatics from the University of Phoenix and a Bachelor of Science in Business Management from Upper Iowa University. She has also attended the Harvard T.H. Chan School of Public Health for continued coursework in executive professional education and leadership strategies for information technology in healthcare.

Brian Collien, Vice President, Managed Care & Large Group Business

Brian Collien joined Unity in 2002. Previously, he provided leadership and business development for both health care providers and health care insurers.

In his role at the combined organization, Collien is responsible for providing vision and leadership for the Large Group line of business and Provider partnerships.  His team develops and maintains the network of hospitals and physician groups that provide high quality and cost effective care to our members throughout the entire service area. He works collaboratively with UW Health, Gundersen Health System as well as a range of ​independent health systems to develop ​innovative programs that provide high quality health care to members. The growth of the Large Group line of business is based on the development of a value proposition that leverages cost-effective health care and services through a full suite of fully-insured and self-funded products supported by a responsive and knowledgeable sales team.

Collien earned a Bachelor’s degree in Finance from the University of Wisconsin – Eau Claire and a Masters of Business Administration from Edgewood College.

Patrick Cory, Pharmacy Director

Patrick Cory has more than 20 years’ experience in managed care pharmacy benefit management. He led Unity’s Pharmacy Program since 2000. Previously, Cory served as a clinical pharmacist in cardiology and critical care.

As Pharmacy Program Director, Cory is responsible for managing all aspects of Unity's Pharmacy program, including medication use policy and formulary management benefit design, financial and clinical reporting, pharmacy network oversight, communications, quality initiatives, NCQA and regulatory compliance.

Cory ​also serves as the Manager of Managed Care Pharmacy Programs at the University of Wisconsin Hospital & Clinics Pharmacy Department and as clinical instructor at the University of Wisconsin School of Pharmacy.

Cory earned both his Bachelor of Pharmacy and Doctor of Pharmacy degrees from the University of Wisconsin – Madison. He completed his Pharmacy Practice Residency at the University of Wisconsin Hospital & Clinics. Cory has been involved with the Pharmacy Society of Wisconsin, recently wrapping up a seven-year stint on the Board of directors, including one-year terms as President and Board Chair during that time.

Kimila Daniels, Vice President of Human Resources and Organizational Effectiveness

Kimila Daniels joined Unity in 2013. She brings more than two decades of Human Resources and Organizational Development experience to the organization. Daniels’ background includes leadership positions in both the public and private sectors as well as a range of industries including property and casualty insurance, entertainment and higher education.

In her role as Vice President of Human Resources and Organizational Effectiveness, Daniels is responsible for Human Resources and Facilities. In addition, she leads strategic human resources, internal communication and organizational management initiatives that drive the organization's growth as an employer of choice in central Wisconsin.

Daniels received a Bachelor's degree in Organizational Leadership from Greenville College and Master's of Management and Organizational Development from Benedictine University in Lisle, Illinois. She holds a Senior Professional in Human Resources (SPHR) certification through the Society for Human Resource Management and is a certified change management practitioner through Prosci® and a certified life coach.

Jennifer Woomer DinehartAssistant Vice President of Marketing

Jennifer Dinehart began her career with Unity in 2013. Her background includes more than 25 years of strategic brand management, including marketing and communications experience in a wide range of industries. She has been an entrepreneur – starting and merging several creative and consumer-driven companies – and began her career as a financial reporter.

In her role as Assistant Vice President of Marketing, Dinehart spearheads strategic market development and provides leadership for traditional and digital marketing and communications activities.

Dinehart received a BA in Journalism from Michigan State University, East Lansing, Michigan and has extensive graduate coursework in Finance from American University, Washington, D.C. She holds Advanced Certificates in Professional Sales, Marketing and Distribution, Export Marketing, Sales, and Operations and Marketing Products Worldwide from the University of California; Interaction Communications from the University of Pennsylvania; and Writing for Business from the Wharton School of Business.

Jeremy Fejfar, PharmD - Director of Clinical Pharmacy

Jeremy Fejfar began his career at Gundersen Clinic Pharmacy in 2003, and transitioned to Gundersen Health Plan in 2012. At the combined organization, Fejfar is involved in all aspects of the pharmacy program, with a focus on daily business done in the West campus location.  This includes communications, medication use policy, quality and clinical programs (including medication therapy management [MTM]), and Medicare Part D.

Fejfar chairs the Health Plan Pharmacy and Therapeutics committee for the West campus, reviews Pharmacy Prior Authorizations for members, and is responsible for many aspects of the pharmacy benefit and clinical programs related to medication therapy for Gundersen Health Plan.

Fejfar earned Bachelor of Pharmacy and Doctor of Pharmacy degrees from South Dakota State University. In addition to his contributions to the combined organization, Fejfar works one day per week at the Gundersen Clinic Pharmacy in La Crosse, Wisconsin, filling prescriptions for employees and patients of Gundersen providers.



Robert Fendt - Director of Clinical Services, Medical Management

Robert Fendt began his career at Gundersen Health Plan in 2009 as Director of Clinical Services. Fendt is responsible for oversight and accountability for clinical care management programs, process and outcomes. This includes overseeing Utilization Management, Case Management, Disease Management and the Pharmacy department.

Prior to his present position, Fendt was a Project Manager for Gundersen Lutheran, the Executive Director of Gundersen Lutheran Home care and the Director of its Hospice Program.

Fendt earned his Bachelor’s of Science degree in Nursing from Viterbo College in LaCrosse, Wisconsin and has a Master’s Certificate in Project Management from the University of Wisconsin, Madison. He has served in leadership roles for the Hospice Organization of Wisconsin and has been a key contributor to a range of national clinical organizations.  


Jim Hiveley, Vice President, Chief Financial Officer and Treasurer

Hiveley began his career with Unity Health Insurance in 2007. He leverages more than 25 years of HMO financial experience, including extensive background in the Midwest regional health insurance marketplace. Hiveley’s experience includes accounting, corporate tax, premium billing and collection, financial analysis and reporting, data warehousing, and internal and external audits.

In his role at the combined organization, Hiveley oversees financial operations and reporting, internal audit, risk management, treasury management and monitoring or internal controls. Hiveley also ensures that the organization is in full compliance with all state and federal regulatory financial reporting requirements.

Hiveley earned a BA in accounting from Augustana College in Rock Island, Illinois.


Karla Lord, Assistant Vice President, Operations

Karla Lord is responsible for business operations functions including Government Claims Administration, Government Enrollment, Training, Amysis System Configuration, Special Investigation Unit (SIU) & Fraud, Waste and Abuse (FWA) functions.

Lord previously served in a range of roles at Gundersen Health Plan, including Director of Business Operations, Compliance Manager / Officer from 2005 – 2008 and Research Analyst in the Plan’s Provider Network Management department from 2001 – 2005.

Prior to joining the Gundersen Health Plan, Lord held several roles over a span of 15 years in Patient Business Service for Gundersen Health System.

Lord holds a BS in Business with a specialization in Management and Leadership from Capella University, Minneapolis, Minnesota.


Gail Midlikowski, Vice President and Chief Operating Officer

Gail Midlikowski began her career with Unity Health Insurance in 2000. She brings more than 30 years of experience in group insurance, HMO operations and TPA services.

Midlikowski is responsible for operational activities including Customer Relations, Customer Service, Business Services, Technical Services and Project Management and Training for all lines of business. Customer Relations is a mission-critical function and includes enrollment, claim processing and customer service. Midlikowski provides leadership that helps ensure that Unity Health Insurance, Quartz® and Gundersen Health Plan exceeds customer expectations in every interaction.

Midlikowski’s extensive experience encompasses every aspect of health insurance operations. Her background includes customer service, claims adjudication, billing and collection, member and provider services, and administration of managed care plans. She also has executive-level experience with merger integration, business sponsorship of IT systems development, projects and implementations.

Mary Pak, MD, Medical Director

Dr. Mary Pak joined the organization as Unity’s Medical Director since 2006. Dr. Pak is responsible for quality improvement, utilization management, accreditation, credentialing and professional relations with the provider community with the other medical directors and contributes to the strategic direction of the organization's clinical programs.

In addition to her role at the organization, Dr. Pak is a practicing hospitalist at University of Wisconsin Hospital and Clinics and holds the appointment of Clinical Associate Professor in the Department of Medicine at the University of Wisconsin School of Medicine and Public Health.

After graduating with honors from Bryn Mawr College, Dr. Pak received her medical degree from Hahnemann University (now Drexel University) in Philadelphia and completed a residency in Internal Medicine at Georgetown University in Washington, D.C. Dr. Pak has additional training in Anesthesiology from Johns Hopkins University in Baltimore. She is board certified in Internal Medicine.

Rob Plesha, Vice President of Actuarial and Small Group and Individual Business

Rob Plesha began his career with Unity Health Insurance in 2007. Plesha’s background includes expertise in the Affordable Care Act (ACA), strategic planning, risk analysis, underwriting and sales management.

At Unity Health Insurance, Plesha’s primary responsibilities include Actuarial, Underwriting, Pricing and Analytics for all products. He also oversees the Small Group, Individual and Exchange lines of business.

Plesha received a BS in Actuarial Mathematics from the University of Wisconsin – Madison. He is an Associate in the Society of Actuaries as well as a Member of the American Academy of Actuaries. He is a graduate of executive education programs at Harvard University and University of Wisconsin – Madison.

In addition, Plesha has given invited lectures at UW – Madison School of Medicine and Public Health, Edgewood College and the Society of Actuaries. He has served as a risk adjustment subject matter expert for the Wisconsin Office of the Commissioner of Insurance and has authored articles and white papers, including Risk Adjustment in Wisconsin.

Plesha serves on the Executive Leadership Team of the American Heart Association – Madison and the Ronald McDonald House.

Elaine Rosenblatt, MSN, FNP-BC, Director of Quality and Care Management

Elaine Rosenblatt began her career at Unity as Director of Quality and Care Management in 2005. She brings more than 40 years’ experience in the healthcare field as a certified Family Nurse Practitioner at UW Health.

Rosenblatt provides direction in disease management programs, health promotion, worksite wellness and medical management. She also assists with implementing NCQA standards and improving HEDIS® measures.

In addition to her role at Unity, Rosenblatt practices part-time at UW Health – University Station, Internal Medicine Clinic, and is a Clinical Professor at the University of Wisconsin School of Nursing.

Rosenblatt received her Master’s in Nursing at Pace University in New York.

Debbie Schiffman, Assistant Vice President of Operations

Debbie Schiffman began her career with Unity in 1988. She has served in many roles in several departments, including Finance, Health Services, Data Analysis and Operations.

In her role as Assistant Vice President of Operations, Schiffman is responsible for Customer Service, Commercial Claims and Enrollment and supporting functions that include system configuration.

Schiffman earned a Bachelor of Arts in Accounting from Upper Iowa University and holds an Associate’s Degree in medical office management.

Sam Schmirler, ASA, MAAA – Vice President, Actuarial & Business Development Services

Sam Schmirler is responsible for actuarial functions within the health plan and as well as in support of Gundersen Health System (GHS). He has also been a key participant in product development and new strategic partnership efforts.

Schmirler joined Gundersen Health Plan in 1998 as Manager of Medicare and Product Development, later serving as Chief Actuary and Director of Provider Network Management. Previously, he served in health actuary roles with Midwest Security Insurance Company, Employers Health Insurance Company and Northwestern National Life Insurance Company. He also held the position of Director of Analytic Services with MEI Inc. (a consultant to large self-funded employer groups). From 1996 through 1999 Schmirler worked on a U.S. Agency for International Development (USAID) funded project designing and implementing health care financing reform within the Moscow Oblast in Russia.

Schmirler holds a Bachelor’s in Mathematics from the University of Wisconsin, Stevens Point (1987) and a Master’s in Theological Studies from Ave Maria University (2002). He is an Associate of the Society of Actuaries (1993) and a Member of the American Academy of Actuaries (1995). Schmirler was ordained a Deacon in the Roman Catholic Diocese of La Crosse in 2007 and for the past nine years has served in diaconal ministry in four parishes in rural Monroe County.


Christine Senty, General Counsel, Vice President and Secretary

Christine Senty leverages in-depth knowledge and expertise in health care reform, corporate governance, regulatory compliance and legislative affairs that strengthen the organization's efforts for growth and continued success.

As General Counsel, Senty is the senior officer responsible for leading the organization's legal team to ensure compliance with laws and regulations. In addition, Senty is responsible for a wide range of legal work and is involved in corporate governance with board committees and the Board of Directors.

Senty earned a Juris Doctor degree with honors from Hamline University School of Law in St. Paul, Minnesota and Bachelor of Science in Business Administration and Human Resource Management from the University of Wisconsin – La Crosse. Senty is involved in numerous professional associations and a member of the Friends of Henry Vilas Zoo Board of Directors.In addition, she serves as the co-Vice President of the Association of Parents and Teachers at her sons' school in Madison.

Greg SkempVice President, Sales & Provider Network Management

Greg Skemp began his career with Gundersen Health Plan in 2007. He provides sales leadership and strategy for Small Group, Medicare Advantage, Medicare Supplement, Individual Market for combined organization and Large Group and Self-Funded for Gundersen service area. He also shares responsibility for broker relationships across the organization.

Previously, Skemp managed the Central Minnesota office for a large Minnesota-based health insurer. He has been engaged in the health insurance and managed care industry since 1993, primarily in Sales and Account Management.

Skemp earned his undergraduate degree in Business (Marketing) from St Cloud State University in St. Cloud, Minnesota. He is presently working toward a Master of Business Administration from St Thomas University in St. Paul, Minnesota.

Skemp holds a license for  Life / Health Insurance in Iowa, Minnesota and Wisconsin. He is actively involved in the local community through youth sports coaching and as a member of Onalaska High School Athletic Booster Club Executive Committee. He provided health insurance educational seminars at Gundersen Health System training sessions for new Physician Assistants and has presented Affordable Care Act education to the local Economic Development Corporation.



Kelly SkiftonAssistant Vice President, Compliance & Government Regulatory Operations

Kelly Skifton is responsible for developing and implementing an effective organizational compliance program. Skifton directs and coordinates efforts to ensure compliance with state, federal, and other regulatory agencies laws, regulations and policies to help prevent fraud, waste and abuse.

Skifton has more than 20 years of industry experience serving both healthcare providers and health care insurers. Prior to her current role, she served as Director of Compliance for Gundersen Health Plan since 2009. Previously, she held a leadership role in revenue cycle for Gundersen Health System.

Skifton holds a Bachelor’s Degree in Business Management and a Masters of Business Administration with an emphasis in Organizational Development, both from Upper Iowa University.  She is certified in Healthcare Compliance (CHC) through the Health Care Compliance Association and is also Epic-certified in Resolute Hospital Billing Insurance and Self-Pay Follow-up.
