Unity Health Plans Insurance Corporation (Unity) is a managed care organization located in the Madison area (Sauk City, Wisconsin).

We are a wholly-owned subsidiary of University Health Care, an affiliate of University of Wisconsin Hospital and Clinics and the University of Wisconsin Medical Foundation.

Unity provides employers Health Maintenance Organization (HMO), Point-of-Service (POS) and Preferred Provider Organization (PPO) plans, including Health Savings Account-qualified plan designs, to approximately 81,000 members throughout 20 counties in southwestern and south central Wisconsin. We also offer State of Wisconsin employee plans.

Individual Health Insurance
Personal Options provides individuals, children (age 2 to 17) and families HMO health insurance benefits comparable to those offered through an employer plan.
