UW Health Welcome Center

Your health is important, so when you need to change health care providers, you want to know that someone is there to help. 

The UW Health Welcome Center is a team of nurses, social workers and primary care referral specialists available to help new Unity members select primary care physicians and transition their care to UW Health.  The UW Health Welcome Center can also help members who have been displaced because their primary care physician has left UW Health.

As a new Unity member and a new UW Health patient, the UW Health Welcome Center will:

• Help you select a new primary care physician (PCP)
  
  
• Work with you to transfer your medical records from your previous health system
  
  
• Gather your medical history
  
  
• Discuss any chronic and/or preventive issues or needs you may have
  
  
• Help you schedule appointments if there is an immediate need
