
Certain covered services under your plan require prior authorization in order for the services to be considered for payment. This means your physician must obtain approval from Unity before you can obtain these services.
Prior authorization lists differ depending on what type of Unity health plan you have. If you are not sure what type of plan you have, please refer to your Unity ID card. The plan type is indicated at the top of your ID card.

Please contact your physician to initiate the prior authorization using Unity's Prior Authorization Request form. To verify that Unity has approved the request for Prior Authorization before you receive the service, please log into MyUnity at unityhealth.com or contact Unity Customer Service through Ask an Expert within MyUnity or call 800-362-3310.
