UW Health Welcome Center
Unity partners with the UW Health Welcome Center, a team of nurses, social workers, and primary care referral specialists, to help new Unity members who select a UW Health primary care physician (PCP) and to ease their transition to UW Health. In addition to helping select PCPs, the UW Health Welcome Center will facilitate transfer of your medical records from your previous provider, gather medical history, discuss any health issues you may have, and help you schedule appointments if you have an immediate need.
