State Medical Benefits

Unity Health Insurance administers your health insurance benefits, which are called Uniform Benefits, and are determined by the Group Insurance Board. The Department of Employee Trust Funds (ETF) publishes a book named It’s Your Choice: Reference Guide which contains the complete description of Uniform Benefits, plus much more information.

Uniform Benefits & Schedule of Benefits

Your benefit information is dependent on which type of employee you are.

1.  State Employee, UW Graduate Assistant, State Annuitant/Continuant, or UW Hospital & Clinics Employee

2.  Local Government Employees

This information changes periodically as participating providers and practitioners join or leave the network, move, or retire. Prior to receiving services, you should always verify that the practitioner or provider you choose is still participating with Unity and accepting new patients.

State Health and Wellness Program

Beginning January 2013, covered State and local members, and their spouse or domestic partner can earn even more rewards for leading a healthy lifestyle through Unity’s new wellness incentive program. By participating in different health and wellness activities, you can earn a total of $150. Learn more. These rewards are in addition to any Fitness First & More reimbursements.


Vision Benefits

Unity covers one routine eye exam per calendar year when received from a participating provider. Non-routine eye exams are covered as medically necessary. Contact lens fittings are not part of the routine exam and are not covered. Unity also does not cover eyeglasses and corrective contact lenses, except for the initial lens per surgical eye directly related to cataract surgery.
