State Medical Benefits

Unity Health Insurance administers your health insurance benefits, which are called Uniform Benefits, and are determined by the Group Insurance Board. The Department of Employee Trust Funds publishes a book named It’s Your Choice: Reference Guide which contains the complete description of Uniform Benefits, plus much more information.

Uniform Benefits & Schedule of Benefits

Your benefit information is dependent on which type of state employee you are.

1.) State Employee, UW Graduate Assistant, Annuitant/Continuant, or UW Hospital & Clinics Employee

2) Local Government Employees

This information changes periodically as participating providers and practitioners join or leave the network, move, or retire. Prior to receiving services, you should always verify that the practitioner or provider you choose is still participating with Unity and accepting new patients.


Vision Benefits

Unity covers one routine eye exam per calendar year at a participating provider. Non-routine eye exams are covered as medically necessary. Contact lens fittings are not part of the routine exam and are not covered. Unity also does not cover eyeglasses and corrective contact lenses, except for the initial lens per surgical eye directly related to cataract surgery.
