Group PPO Plans
Unity’s PPO plans offer two levels of benefits: In-Network and Out-of-Network. With the PPO plan, your employees don’t need to choose a primary care physician. When services are received from In-Network providers, benefits are paid at a higher level.

Eligibility Guidelines
To be eligible for the PPO plan, you must meet the following guidelines:
- Your company must be located in Unity's 20 county service area
- If you select the PPO plan, it must be paired with the Unity HMO
- Your company must have a total of 6 or more enrolled employees
- Your employees must reside outside of the Unity service area OR in an eligible state.
Provider Networks
To offer a statewide and national PPO network, Unity contracts with HealthEOS (Wisconsin) and PHCS MultiPlan (for other states).
The HealthEOS network includes hospitals, clinics and physicians throughout Wisconsin. The PHCS MultiPlan network provides access to health care services nationwide.
