Group Point of Service (POS) Plans

Unity’s Point of Service (POS) plans are similar to an HMO, but with added flexibility.  Specifically, employees can receive care from in-network or out-of-network providers without a referral from their Primary Care Physician. However, benefits for care received out-of-network will be paid at a lower benefit level.

Eligibility Guidelines

• For groups with 2-5 enrolled employees, your company must be located in Unity’s 20 county service area.
  
  
• For groups with 6 or more enrolled employees, your company must be located in Unity’s 20 county service area to qualify for the POS plan or any of the multi-plan offerings:

  • HMO & POS & PPO

Provider Network

Unity’s POS network includes the same providers as the HMO network. In addition, employees have access to health care providers throughout Wisconsin and the United States.  Learn more about the Unity provider network.
