Fitness First

Unity's Fitness First Program is an exercise reward program for members and their spouses or domestic partners enrolled in the health plan. Members and spouses or domestic partners can earn either a combined total of $200 each calendar year, or they can choose to exercise for prizes.

Choose your reward
Decide if you would like to work out for cash or prizes. Read the information below and follow the instructions for the reward you select. You must be enrolled in the Fitness First program for your workouts to count toward a reward.

Cash
You and your spouse or domestic partner can earn a combined total of $200 per calendar year in one of three ways:

Subscriber earns $200 in two $100 increments: one after six consecutive months and the second after another six consecutive months ($100 every six months)
Spouse or domestic partner earns $200 in two $100 increments: one after six consecutive months and the second after another six consecutive months ($100 every six months)
Subscriber and spouse or domestic partner split the $200; each earn $100 after six consecutive months

To earn the cash reward, you must:

Enroll in the program using the Fitness First Enrollment Form
Work out a total of 60 times in a six consecutive month period at a participating health club.
Log in at your health club each time you work out
Work out at least 30 minutes each time
Participate in a qualifying activity at your health club such as bicycling, running, swimming, walking or weight training
Record only one workout per day

To earn prize rewards you must:

Enroll in the program using the Fitness First Enrollment Form
Record your workouts on the Fitness First Tracking Form
Redeem your tracking form within 12 months of the earliest workout date, but no later than 30 days after the last recorded date.
Work out at least 30 minutes each time
Participate in a qualifying activity such as bicycling, running, skiing, swimming, walking or weight training
Record only one workout per day

Choose your Prize , Sign the Tracking Form, and Mail to Unity when you’ve completed enough workouts for the prize of your choice.

BadgerCare Plus members are eligible to receive one prize per calendar year from this list of prizes.
