Receive Your Postpartum Check-up and Receive a $15 Gift Card!

What is a postpartum check-up?

The American College of Obstetrics and Gynecology recommends a postpartum check-up for both vaginal and cesarean births. During the postpartum check-up, your practitioner:

Will make sure that your body has recovered from pregnancy and birth and that you are not having any problems
Will check your weight and blood pressure, examine your breasts and abdomen, and perform a pelvic exam
May also speak with you about postpartum depression or other concerns of depression you might be experiencing

To monitor your recovery from pregnancy and birth, the American College of Obstetricians and Gynecologists (ACOG) and Unity encourage you to schedule your check-up within 4-6 weeks after delivery. As an incentive, Unity offers a $15 gift card to either Target or Wal-Mart for receiving your postpartum check-up.

How do I obtain my postpartum check-up gift card?

If you did not receive a form in the mail, then print, complete and send in the Postpartum Check-up Postcard
Bring it to your postpartum check-up *
Have your health care practitioner sign the postcard
Check which store gift card you prefer
Send the completed card back to Unity Health Insurance at: Attn: Health Services RP, Unity Health Insurance, 840 Carolina Street, Sauk City, WI 53583

*In order to receive a $15 gift card, your postpartum check-up must occur within 4-6 weeks following delivery, and you must be a Unity member at the time of the check-up. This reward may not be combined with the 9 Months & More postpartum reward.
