Asthma Program
Unity wants to help you achieve control of your asthma with the Asthma Health Management Program.
Asthma Health Management Program participants* receive:
- Informational mailings with tools and tips for managing asthma
- Important reminders, reference materials and resources
- Annual flu vaccine reminder
- Articles on asthma in the Unity Member Newsletter
- A member satisfaction survey about the asthma program
*Participants may opt-out of the program at anytime by contacting Health Services at 866-884-4601 ext. 54894 or asthma@unityhealth.com.

You can help control your asthma by practicing asthma self-management skills. Self-management includes things you can do to minimize your asthma symptoms. By learning about your health condition, you become more involved in your treatment. Listed below are some self-management skills to practice.
- Visit your asthma practitioner twice a year, even when you are feeling well and your asthma is under good control.
- Take the Asthma Control Test and share the results with your health care practitioner.
- Fill out the Asthma Action Plan with your health care practitioner.
- Make a list of questions to ask your health care practitioner at your next visit. See a list of questions you may have.
Living Well With Chronic Conditions Program
The Living Well with Chronic Conditions Program teaches people with chronic conditions to better manage their health.
