Community Supported Agriculture (CSA)

Reimbursement for Eating Locally Grown Fruits & Vegetables

You can support a FairShare CSA Coalition member farm by purchasing a produce share that entitles you to a “share” of the season’s harvest and receive reimbursement from Unity. Unity will reimburse you up to your Fitness First & More program maximum. Your FairShare CSA Coalition member farm produce share will be reimbursed in the same calendar year in which it was purchased. 

Shareholders support the farm by purchasing a vegetable or fruit share that entitles them to a share of the season's harvest. Produce shares vary in cost depending on produce type, length of season and individual farm.  Meat, egg, and dairy CSA shares are not eligible for reimbursement.

Requirements for Reimbursement:

• Purchase a produce share from a FairShare CSA Coalition member farm
• Be a Unity member on the date of purchase
• Complete the Reimbursement Form
• Mail the completed reimbursement form and one of the following proofs of payment to Unity in the same calendar year it was purchased:
• • Itemized receipt from the farm
  • Copy of an email from the farm acknowledging your purchase.  This must include the amount you paid for your purchase and date of purchase.  Please note the date of the email does not constitute the date of purchase.
  • A copy of your canceled check or credit card receipt

The Fitness First & More program is made available as a courtesy to our members and it is not part of your health insurance coverage. This program is not a contract between Unity Health Insurance and participants and may be discontinued or changed at any time without notice.
