Fitness First for Kids

Fitness First for Kids is an exercise reward program for kids between the ages of 5 and 18 who are insured dependents of a Unity member. Kids can earn prizes for being physically active a minimum of 30 minutes a day.

To earn a prize reward your dependent must:

Be between the ages of 5 and 18
Be an insured dependent of a Unity subscriber
Record their activities on the Fitness First for Kids Tracking Form
Be active a total of 30 minutes each day
Participate in a qualifying activity such as bicycling, running, swimming, soccer and even activities that are part of their physical education class
Record only one activity per day
Submit the tracking form within 12 months of the earliest recorded date, but no later than 30 days after the last recorded date.

Requirements for Reimbursement:

Enroll - Complete the Fitness First For Kids Enrollment Form
Track your Workouts
Choose your Fitness First for Kids prize, sign the tracking form and mail it to Unity
