Group HMO Plans for employers with 2 or more enrolled employees
Unity’s Group HMO plan is a managed care plan that provides group health care coverage through a defined network of doctors, hospitals and other medical professionals. Our provider network is built on the foundation of the University of Wisconsin Hospitals and Clinic physicians, clinics and hospitals. Unity is owned by UW Health, and we offer a broad range of providers within the UW system in addition to local community providers.
To offer a Group HMO plan, an employer must be located in Unity’s 20 county service area and 2-5 employees must enroll in the plan. For groups with 6 or more enrolled employees, you may offer a PPO or POS plan in addition to the HMO plan. Please note that some procedures require prior authorization [PDF]. This means that the treating physician must obtain approval from Unity before the service will be authorized for coverage.

Fitness First and Fitness First for Kids programs are independent of the group HMO policy and there is no extra employee cost involved.
We even work with employers to develop tailored Employer Wellness program that address an organization’s specific needs. Program features include health risk assessments, health screenings, educational sessions, group consultations and custom reports.
